How to prevent sagging cheeks after the cheekbone reduction?

There is a question my patients ask when they visit us for the cheekbone reduction. 
Many people ask how to minimize and prevent the sagging of the skin.

Theoretically and empirically, a way to minimize sagging cheeks is depends on surgical methods. 
Although it is important to select a surgical method that fits the patient's condition, the degree of sagging may vary depending on the surgical method.

Let me explain some of the most representative surgical methods known.
For convenience, I will call the 45-degree cheeks and side cheeks as a “body” and “arch”, respectively.

1. L-shaped osteotomy 

L-shaped osteotomy is literally an L-shaped incision. A surgical method is cutting the front 45 cheekbones into an L shape and pushed them backwards. 

Although it is similar to I-shaped osteotomy, the advantage of L-shaped incision is that it prevents the cut cheekbones from drooping down from the longitudinal ablation line of the I-shaped incision, which can be held steady, and the raised 45-degree cheeks can be pushed back.

2. I-shaped osteotomy 
I-shaped osteotomy, also known as a straight incision, is a form of cutting bones removing it, The I-shaped bone is pushed back into the inner part.
I-shaped osteotomy is often fixed with wires and sometimes non-fixed.
It's as important to hold the bones as well as the fixation, but non-fixed methods may not be suitable for active people.

3. II shaped osteotomy
you all know that there is a high chance that the skin that has been removed from a cheeks will become sagging. The II shaped osteotomy is a method of removing the fractured part by cutting it by 4 to 7mm in order to have an effect in the event of a high protrusion of the arch and body.
However, the more the cheekbones move inward, the more likely it is that the flesh of the deleted bone will fall.
